Application for a Passport
IMPORTANT. Read instructions carefully before completing the form.
A member of the applicant's immediate family is not acceptable as a recommender.
HOW TO COMPLETE FORM
(A) All applicants should complete Sections 1, 2, 6, 7 and if appropriate to their own application, Sections 3 and 4.
(B) Children under 16 years of age can normally be included (without extra charge) on the passport of the parent who, in completing the application form must also enter particulars of the children under Sectiion 5. The consent of the legal guardian required for the inclusion of a child on the passport. Application for the addition of children to an existing passport should be made by completing a Child Addition form.
Should separate passport be required for a child under 16, application by or with the written consent of the legal guardian can be made on Form "B".
(C) Signing the form. The application form must be signed in Section 7 by the applicant. Section 8 should be completed by the person verifying the declaration who should be a Member of the National Assembly, Island Assembly, Justice of the Peace, Notary Public, Minister of Religion, Medical or Legal Practitioner, Senior Civil Servant, Senior Bank Official, Gazetted Police Officer, or any person of similar standing PERSONALLY ACQUAINTED with the applicant.
The Fee for the Passport is one-hundred dollars ($100.00) (payable in Stamps) for 10 years.
DOCUMENTS TO BE PRODUCED
(D) (i) All applicants should produce birth certificates or certificates of naturalization or registration as citizens of St Christopher (St. Kitts) and Nevis as the case may require.
(ii) Married women (including widows and women whose marriages have been terminated) should produce documents specified in (i) above along with Marriage Certificate and applicants who are divorced should produce evidence of divorce. A married woman claiming citizenship by marriage should also submit the birth certificate of her husband.
(iii) For children under 16 to be included in the passport, the children's birth certificates are required. If the applicant is not the legal guardian a letter of consent from the father/mother or other legal guardian or guardian is required.
Documents as for all applicants (see paragraph (i) above; the written consent of the legal guardian is also required, except where applicant is married.
NOTE:- Where an Order or Direction has been made by the High Court or in chambers, or by a Magistrate regarding the custody of a child such Order must be produced or the nature of direction stated.
(iv) Change of Name. If the applicant has changed his or her name, the deed poll recording the change or other evidence to show how and when it was changed must also be submitted. A Birth Certificate with note of change of name is regarded as evidence that a change of name has been effected.
(v) Persons born outside of St. Christopher (St. Kitts) and Nevis as constituted on 19th September, 1983 and all persons claiming Citizenship by naturalization or registration, must complete Section 4 and may be required to produce documentary evidence in support of the statements made therein.
(vi) A Police Certificate should also be produced.
Three copies of a recent photograph of the applicant must be included with the application. These photographs should be taken full face, without a hat and must not be mounted. The size of the photographs must not be more than 2½ inches by 2 inches or less than 2 inches by 1½ inches. The photographs must be printed on normal thin photographic paper and must not be glazed on the reverse side. The recommender is also required to endorse the reverse side of one copy of the photographs with the words: "I certify that this is a true likeness of the applicant Mr./Mrs./Miss " and add his signature.
